Video case study: Comprehensive geriatric assessment in a community setting

The second video case study in our two-part series on frailty shows how using a nurse-led approach to assessment can improve health outcomes for older people living in the community.

This is the second in a two-part series that presents video case study examples of the use of comprehensive geriatric assessment in different clinical settings

Produced as part of our resource collection on frailty, the second case study shows how using a nurse-led systematic approach to assessment can help avoid hospital admissions and improve health outcomes or quality of life for older people living in the community.

The case study presented explores the application of CGA with an 82-year-old adult living at home with multiple health issues.

Using CGA, Clifford Kilgore, nurse consultant for intermediate care/older people at Dorset HealthCare University NHS Foundation Trust, demonstrates how nurses can respond to, and manage, older people living with frailty in the community and he identifies interventions that the frail older patient can benefit from.
