Produced as part of our resource collection on frailty , the second case study shows how using a nurse-led systematic approach to assessment can help avoid hospital admissions and improve health outcomes or quality of life for older people living in the community.

The case study presented explores the application of CGA with an 82-year-old adult living at home with multiple health issues.

Using CGA, Clifford Kilgore, nurse consultant for intermediate care/older people at Dorset HealthCare University NHS Foundation Trust, demonstrates how nurses can respond to, and manage, older people living with frailty in the community and he identifies interventions that the frail older patient can benefit from.
