Teachers are increasingly worried about the complexity of looking after a child who has diabetes. Counting carbohydrates, differing insulin regimens and dealing with hypos were also difficult for clinicians. Different people were using different tools and none was easy to reach in an emergency situation.
Led by nurse specialist Sandra Singleton the team developed the Individualised Health Care Plan for a Child or Young Person who has Diabetes.
It is quick and simple for the specialist paediatric diabetes nurse to complete with the child or young person and their family. The plan can then be forwarded to all the education settings involved.
It is suitable for any child or young person who has diabetes, irrespective of age, type of diabetes, regime or educational setting.
The care plan was trialled in three NHS trusts and audited, then rolled out nationally via the networks.
Diabetes UK asked the team to adapt the care plan to be a standalone document, which is now available through their online resource.
