<p>Working in the private sector as a clinical lead and trainer, I read the CPD article with great interest. As the article acknowledges, assessment frameworks provide a structure to ensure all members of the team caring for a patient have the same goal.</p>

<p>I care for many people with coexisting conditions, including asthma, in a nursing and residential care home. The CPD article helped me to change practice in this setting.</p>

<p>I work in a nursing home and care for older patients who are at increased risk of falls. Most patients are taking multiple medicines, experience age-related physiological changes and have medical conditions, which may make them more susceptible to falls. In addition, some patients have dementia, Parkinson’s disease or have had a stroke.</p>
