Insulin therapy is needed by around 20 per cent of people with type 2 diabetes. Traditionally, insulin was started in a secondary care setting, and patients were looked after in secondary care diabetes clinics. This picture is rapidly changing, and in some areas starting insulin in primary care is now the norm. This article explores some of the issues associated with starting insulin in patients with type 2 diabetes.
