What is an urgent community response team?
Urgent community response (UCR) teams provide rapid response care to people in their homes to avoid hospital admissions and enable people to live independently for longer.
Through these teams, older people and adults with complex health needs who urgently need care can access a range of health and social care professionals within two hours. UCR teams often include physiotherapy and occupational therapy, prescribing and reviews, and social care, says NHS England.
