The Medically Fit for Discharge Ward Team
This nurse-led team is improving the experience of care for patients unable to be discharged into the community due to lack of places.
When these patients were on an acute ward with seriously ill people, they received less attention from staff, delaying and compromising their rehabilitation and further preventing timely discharge.
The development of the new ward was led by associate director of nursing Maddie Groves. A protocol was designed stating who would be referred and when, and a patient information leaflet explained the ward’s focus.
Some people assigned to residential care have instead been able to go home thanks to extensive rehabilitation or therapy on the ward, and fewer patients need rehabilitation at community hospitals.
In a recent evaluation, 98% of patients said they felt they had been treated with respect and dignity. Meanwhile, the initiative has freed up qualified nurses, medical practitioners and other staff to concentrate on sick patients.
