Nursing Older People Award finalist: Care Home Support Team

This team is improving care home residents’ health through a comprehensive staff training programme and ‘ward rounds’. Training focuses on areas including falls and pressure ulcer prevention, dementia awareness, hydration and nutrition and end of life care. Ward rounds enable clinical supervision of staff, supporting them to embed training into practice. There has been a huge increase in the use of advanced care plans to ensure residents are managed in their preferred place of care. Staff are also detecting signs and symptoms of infections earlier. The project has reduced A&E attendance by 12% and hospital admissions by 21%.
