a joint practice nurse/community learning disability nurse annual health check for primary care patients with learning disabilities

In view of the complex health and communication difficulties that can affect patients with learning disabilities, a general practice piloted an annual health review with the practice nurse and a learning disabilities nurse. Here, Graham Martin and colleagues report on the success of this patient-friendly service

Moves are currently under way to provide better, more rational and improved services for people with learning disabilities. This is clear from Valuing People (DoH 2001) and from various other Department of Health directives (Lindsay 1998, Lindsay and Russell 1999). The relocation of learning disability services in the community and the intention that all people with learning disabilities should be registered with a general practitioner signals a clear intention towards joint primary care/community learning disability team working.
